Certificate of Insurance Request
Certificates of insurance may be required by contract in several areas throughout campus. Contracts for the rental of an event venue, lease of a vehicle or equipment, or the award of a research grant all contain insurance requirements which terms may be met by the issuance of a certificate.
Certificates of insurance are issued and maintained by the Risk Management and Safety Department (RMS). Effective January 1, 2017, In order to obtain a Certificate of insurance, the Certificate of Insurance Request Form should be completed and submitted to Barb Davey in RMS.
Once insurance requirements are reviewed and accepted, the appropriate paperwork will be scanned back to the requesting individual.
