Incident/Claim Reporting

Automobile

Incidents or claims involving University owned, rented, leased, or courtesy vehicles should be reported through the Automobile Loss Notice Form.

Claims may include injury to persons or damage to vehicles. 

In the event of an incident or accident, departments and individuals are responsible for following the Risk Management and Safety Automobile Accident Procedure (125 kb, pdf).  An overview of the procedure (109 kb, pdf) is also available for your reference.

Departments or divisions are responsible for a $500 deductible for damage to University owned, rented, leased or courtesy vehicles.

Incidents or claims involving Valet parking at the Morris Inn should be reported through the Automobile Loss Notice and are subject to a $2500 or $3000 deductible.  The Valet Service Automobile Claims Process (44 kb, pdf) and the Valet Auto Reference Form (77kb, pdf)  are available for your reference.

General/Excess Liability

Incidents or claims involving bodily injury or damage to the property of guests or visitors of the University must be reported through the Incident Loss Notice Form.  Claims include incidents such as slip and fall accidents and damage to vehicles in parking areas.

In the event of incident or claim, departments and individuals are responsible for following the General Liability Accident Procedure (121 kb, pdf).  An overview of the procedure (112 kb, pdf) is also available for your reference.

All injuries to employees of the University will be reported through the Worker's Compensation process.

Property

Incidents involving damage to or loss of University property (including property owned by departments or divisions), must be reported through the Incident Loss Notice Form.  Claims may include damage to buildings, furniture, fixtures, computer or other equipment.

In the event of loss, Departments and individuals are responsible for following the Property Claim Procedure (117 kb, pdf).  An overview of the procedure (111 kb, pdf) is also available for your reference.

Departments or divisions are responsible for a $5,000 deductible for damage to or loss of University equipment, including, but not limited to, research equipment, golf carts, computers, books, and fine art. 

Coverage is not provided for property individually owned by faculty, staff or students; regardless of use or location at the time of loss.
